The chances of a lung cancer patient being a smoker are very good. The chances of a smoker being a lung cancer patient are very bad.
~36m smokers in the USA. ~220K new lung cancer patients each year. ~80% of those patients are smokers.
Ergo, smoking and lung cancer aren't directly related. Not all smokers will develop lung cancer.
I have a friend who smokes. I wonder why they haven't developed lung cancer.
Heart disease, emphysema, .... lots of other good stuff besides lung cancer. No one has ever seen the products from combustion of tobacco cause lung cell DNA that controls cell growth and the cell cycle to get whacked. We deal with probabilities. I would tell your friend to stop smoking. Or maybe he just gets killed in a car accident, so all is well.
