What is the usual practice for taking out insurance policies against player injuries? Do teams insure all players or just a select few?

they most definitely insure all the players.

that'd be stupid if they didn't, i think.

You're probably right about the insurance.

After the Amare injury, maybe players should reconsider wearing eye protection during games. James Worthy and Horace Grant proved goggles aren't a hindrance to successful careers. Physical contact to the face and in particular the eyes is a common occurence playing BB. Maybe its vanity on the part of the players or like most young people they feel a sense of invincibility.In any other work environment with with the potential for physical contact to the face, eye protection would have been mandated a long time ago. Why gamble with your career?

IIRC, the NBA has a bulk insurance policy over every player in the NBA. However, the insurance company can pick a certain number of players (10 or 12, I think) that they refuse to cover.
