TNT is a part of most basic cable packages, as are ESPN and ESPN2. NBATV, like HBO for basketball fans, is a premium station you have to pay for in addition to basic cable pacakages. It is, for instance, not available through my cable company in my area. The least popular matchups are relegated to less viewership because it is a smaller audience. Therefore, the NBA doesn't put the marquee matchups on the network because they could draw a larger audience. The average fan is a diehard fan of the NBA and is more likely to watch a less popular matchup. You will never see a Lebron James/Kobe Bryant matchup on NBATV.

The top playoff seeds are often relegated to NBATV, especially if it is a non-competitive matchup. That is why San Antonio and Chicago both have games on NBATV. It is assumed (and both Indiana and Memphis has disproved that thus far) that the games will be blowouts and therefore will not draw a large audience. The top seeds will be seen again when they have a better opponent. Meanwhile, Boston and New York, with a lot of buzz, will never touch NBATV. Denver and Oklahoma City will never be on NBATV because it is seen as a competitive matchup. Portland/Dallas will not be on NBATV for the same reason. The NBA wants to draw as much of an audience as possible because it translates into more money. Therefore, it is a sign of a bad matchup and a boring playoff game to be on the network when the NBA wants to draw as much of an audience as possible. Still, in years past, the games simply would not be broadcast or they would stretch out the first round to broadcast the most games possible. Having NBATV is a benefit, especially if you can afford it and it is available in your area.
