Henry: Portrait of a Serial Killer deserves a mention as well. Great film.
I find something like that much more disturbing than the Human Centipede/Serbian Film variety.

I've seen Henry. The scenes with the family massacre and the guy that tried helping them with their car still disturb me to this day.

I agree. The image that the basic premise plants in your mind, is much more disturbing than the film itself, in my opinion.

I pretty much avoid films that can be described as "disturbing" so my choices will seem pretty lightweight compared to all these others. I had to leave the theater during the epic first scene of Saving Private Ryan, but I eventually got thru it around my 3rd attempt. More recently it was 127 Hours----I had to pause the dvd and go lie down a couple of times to make thru the cutting sequences :lol

Martyrs (French movie) Awesome and disturbing at the same time. On Netflix so give it a watch. Anyone looking for original horror and a crazy concept would be shameful if not viewed.
Inside (Ills) another french horror movie that is amazing as well. Not for the weak of heart.
I Saw the Devil (Korean horror/serial killer movie) Beautifully filmed and acted.
Funny Games (Dutch film and even the American remake was disturbing as hell)
Cannibal Holocaust, I spit on your grave (original), Salo, Hellraiser 1 and 2 all good choices.
David Lynch movies as well.
