A Baton Rouge, La., hospital is closing the only emergency room on the city's impoverished north side, a real-world ripple effect of the ideological clash over President Barack Obama's health care law.

The shutdown on April 1 serves as an early warning for hospitals in states like Louisiana, where Republican Gov. Bobby Jindal turned down federal money to expand the Medicaid program for the poor. Charity hospitals will lose billions of federal aid beginning late next year, a cut that was supposed to be offset as more residents were covered by Medicaid.

In Louisiana, Baton Rouge General's Mid City hospital was already caught in that vise. It was flooded with the uninsured after a nearby charity hospital was closed. Louisiana provided a one-time injection of funds last year from the federal aid program that's about to be cut. With that money gone, the hospital is closing the emergency room.

Let's review the way Medicaid expansion was supposed to work.

First, Medicaid reduced the amount of money hospitals will be reimbursed for providing health care to people whose income falls below a certain level.

At the same time, by expanding Medicaid eligibility, more people will be insured, having two effects. (1) Insurance payments will offset the loss of payments to the hospital; and (2) because people now have medical insurance, they will establish a relationship with a physician thereby staying healthier and avoiding emergency room visits because untreated illness can be caught early.

In (Republican-controlled) states where Medicaid expansion was not enacted, hospitals are feeling a huge pinch and, very likely, this closing in Baton Rouge is only the beginning of a nationwide trend.

In fact, here in Virginia, Lee Regional Medical Center in Pennington Gap -- the only medical center serving a mountainous region with a mostly poor, elderly population -- closed a year ago for the same reason.

The event on June 22 had been reserved for months, according to several city leaders, including Davis Rhorer, executive director of the Downtown Development District. The technology giant IBM voiced "strong opposition" to Louisiana's so-called religious freedom legislation as far back as mid-April.

In a letter to the governor, IBM executives said the legislation cut at the very heart of what their company stands for, equal rights and opportunity for everyone, discrimination for no one. The letter implored Jindal not to support any agenda that protected discrimination against LGBT people.

• The Federal Government pays 100% of expansion costs for the first three years and 90% thereafter until 2022.

• Cost is the most cited reason for not expanding Medicaid. While current Medicaid programs do cost state taxpayers a lot of money, that spending is balanced by unpaid hospital bills and the effect those bills have on the rising costs of premiums.

Although the event was identified as separate from his political responsibilities, Jindal has a significant history of tying conservative religious ideology into his political work. Here are five examples of Jindal’s most controversial legislative and ideological moves:

Approved The Inclusion of Unverified Scientific Theories in Public School Education

The Louisiana Science Education Act (http://ncse.com/files/08_la_sb733-amend.pdf)was signed into action during Jindal’s first year in office. The bill encourages the fostering of “critical thinking” by diversifying science education away from standard scientific consensus into other ambiguously described alternative theories, including allowing educators to provide their own teaching and reading materials at their own discretion.
Although religion is not explicitly stated in the legislation, critics of the bill believe it was as an agent of Jindal’s publicly held desire to further integrate traditional Christian values, particularly through the use of the creation theory. “I think that local school districts, not the federal government, should make the decision about how they teach science, biology, economics,” Jindalsaid (http://www.theguardian.com/us-news/2015/jun/24/bobby-jindal-republican-creationism-2016-election) at a Christian Science Monitor event last September. “I want my kids to be taught about evolution; I want my kids to be taught about other theories.”

The bill also worked to ban future marriage equality legislation. Expresses Anti-Islamic Immigration Sentiments

In January, Jindal received significant backlash (http://www.theatlantic.com/politics/archive/2015/01/the-sophisticated-bigotry-of-bobby-jindal/384997/) for his anti-Islamic immigration sentiments shared on the Family Research Council’s Washington Watch (http://www.frc.org/get.cfm?c=RADIO) radio show. Jindal expressed fear of an Islamic takeover and imposition of Sharia Law in the U.S. “If they want to come here and they want to set up their own culture and values that’s not immigration, that’s really invasion if you’re honest about it,” Jindal said on-air.

Refuses to Enforce Comprehensive Sex Education In State Education Curriculum

Jindal has held his pro-life views close, continuing Louisiana’s history of strict abortion restrictions and few accessible family planning options. According to the Guttmacher Institute (https://www.guttmacher.org/pubs/sfaa/louisiana.html), as of January 13, 2015 women wishing to seek an abortion the state will be required to undergo significant counseling including information intended to discourage the procedure, health insurance provided through the Affordable Care Act state exchange may not cover abortions, and women must undergo an ultrasound, and have the image shown to them, before receiving an abortion. Jindal, who’s also running for president on the Republican ticket, sent a letter to Obama Wednesday urging the president not to mention climate change during his trip to Louisiana.

“Although I understand that your emphasis in New Orleans will – rightly – be on economic development, the temptation to stray into climate change politics should be resisted,” Jindal states in the letter. “While you and others may be of the opinion that we can legislate away hurricanes with higher taxes, business regulations and EPA power grabs, that is not a view shared by many Louisianians. I would ask you to respect this important time of remembrance by not inserting the divisive political agenda of liberal environmental activism.”

This week, Jindal writes in the letter, should be solely about mourning those lost in the storm and celebrating the recovery New Orleans has made so far.

But Obama had a different plan for his time in New Orleans. The trip is the second stop in his 11-day climate tour, a trip that started at the Clean Energy Summit in Las Vegas and will end in Alaska. Along the way, Obama is planning on speaking about the country’s need to invest in renewable energy, protect coastal communities, and be part of a global effort to address climate change.
