Need help for my Uncle ( "team is everything" medals)

The things you get in the news paper ( I think) about the championship plaques...hes trying to find the last one that he needs which is the team is everything motto...I called up hebs an they dont have any but apparently theres some number I can call and order it? any one with advice?
It was the one that celebrated our 4th championship I think...

I finally found Duncan's on ebay. (I only wanted Duncan's.)

Thanks, though I dont think that is the one he is looking for...I think theres a different one that came out not to long ago after the championship. I think ive seen the adds on tv for it.
