Name the only team that didn't make the playoffs. (and guess the coach too!! :D)

I know it was the Raiders, QB'd by Jeff Hostetler....I'll be back with the coach and the year.

EDIT: Nevermind, that Raiders team started 8-2 and then flamed out. I have no clue what the answer is.
