Have the Spurs ever lost a playoff series after being up 3-0?

No NBA team ever has. I think the Mavs should first focus on winning the series with NOH before they make assumptions about meeting the Spurs in the second round. The Spur would also be well served if they ended this quickly to get some rest.

The 1942 Toronto Maple Leafs and the 1975 New York Islanders both came back from 0-3 to win the series.

One rule of thumb is if the Spurs aren't good enough to beat the Suns 4 in a row, Phoenix isn't good enough to beat San Antonio 4 in a row. So, call this one over. Get out your brooms if you don't believe in jinxes.

The 91 of 263 is for all sports. For the first round of the NBA playoffs, teams up 3-0 when they played Game 1 at home are 9-1 in Game 4 and have never lost the series.
NBA Prelim H 9-1 (.900) 10-0 (1.000)
While there are no guarantees in sports (just ask the 2004 Yankees), history is very much on the side of the Spurs now. They have a 90% chance of completing the sweep and are a lock to win the series.
