Specifics of the Lottery

Just wondering about the exact criteriea for the NBA Lottery. Obviously the teams with the worst records have x amount of tickets in the lottery more than the not-so-worst teams and this number descends in order of most losing record.

I was just curious as to:

1. How many teams are in the lottery (forgot)

2. What are the conditions by which a team is admitted to the lottery. (Obviously not make the playoffs, but anything else?)

3. Any other details of the lottery that are not really discussed.
