Do you not know ANYTHING about how insurance works? It's done annually by contract. X amount per month for Y amount of coverage. If Y changes at renewal, the X changes as well.

If the policy is going to be changed, the premiums will pay for the covered items and treatments in the future contracts.
not if they exclude maternity benefits too (which most student policies exclude)
Not enough to hit my radar or for any underwriter to have ever cited it as a reason for any premium adjustment one way or the other, tbh. Again, ironically, considering the national fervor. Birthing babies also insignificant, btw (premature births notwithstanding).
