As far as I know it's not the next leader in votes, it's a coaches' decision. Jermaine still has to go to the All-Star game and participate in all the media stuff, etc.
Manu's been too injury prone this year while Tony's had an awesome season so far. At least it'll be a good time for Manu to get 100% healed and ready for the second part of this season. We're going to need him ready for the playoffs instead.

I think the coaches vote for the next 7, and then from that bunch, a replacement is selected and the Commissioner is then permitted to select a 13th player. I seem to recall a few years ago that a similar situation allowed Vlade Divac to play in the All-Star Game. Someone got hurt and Stern was permitted to choose a replacement. He may be guided by who the fans voted for, or who the players voted for, but I think it's Stern's decision.

Yes. But I think that's the point. The replacement for Jermaine won't be Rasheed, who is the next-highest vote getter; the replacement starter will be one of the forwards voted-in by the coaches -- if Rasheed is voted in by the coaches, he'll probably start. If Rasheed isn't voted in by the coaches, Stern might appoint him to the All-Star team anyway.
We are talking about how Jermaine O'Neal is injured and how the coaches will pick 7 players for the reserves and then Stern will pick players to replace any of the injured starters.
