The Russian Roulette scenes in The Deer Hunter were pretty disturbing to me when I first saw that film.
Oh man I forgot about that one. It's been such a long time, I'll need a rewatch to be sure, but I think the most disturbing thing about that film is the coldness of it all.
+1 on Happiness...that movie is chock fulla disturbance.
Midnight Express has plenty of ed up scenes as well.

I am much more disturbed by the fact that Matthew McConaughey is still making movies than anything on this thread.
Some of the in the Wrong Turn series of films, or maybe just because they all suck major ass.
The scene towards the end in Once Upon a Time in the West where you find out why Harmonica is out for revenge. Frank was pretty evil.
The scene where the "good" brother just gives up and joins his twin in squalor was one of the most psychologically devastating things I've seen on film.
Also the medical "tools".

I couldn't even finish the movie because it was so ing awkward.
Gary Oldman in "The Professional." In particular, the one scene where he corners 12-year-old Natalie Portman in the bathroom. He says some ridiculously sinister to her with a bat crazy look in his eyes. He was an amazing villain in that film.
No amount of violence in a movie ever bothers me... chalk that up to being desensitized, I don't know. But the movie Kids really bothered me for some reason. It's just too real. Inner city kids running around having unprotected sex, raping, passing around HIV... it's disgusting.
Gooood one.
Eraserhead is my #1 all-time pure discomfort movie.
Naked Lunch centipede rape scene is not fun, either.
Maniac has a really gloomy, seedy vibe that's ultimately scarier than any of the Savini-assisted murders.
And another vote for Irreversible. Jesus.

Most disturbing movie i have ever seen, by a very long shot has to be Gummo. Another Harmony Korine movie. About white trash middle america. Movie still creeps me out just watching the trailer here.
I think everyone should see this movie once. It is eye opening to say the least.
The Achilles tendon cut in Pet Semetary. It's not the most disturbing thing but the fact that it kinda catches you off guard is what makes it.
