Doctors would be salaried, no more scams of "fee for service".
Most doctors would opt out and continue fee for service to wealthy clients who would pay cash to the doctors, nurses, clinics.
But those wealthy clients would still have to pay ALL of their income deductions to the national system.
And whatever they pay for for-profit, fee-for-service boutique medicine would not be deductible from their national health payments.
