IMO they got to be somewhere between the 3rd and fifth position. Clearly behind the Rockets and probably behind the Lakers too. The only other two teams that I think could be bigger than the Spurs are the Celtics and Bulls.

Bulls are without a doubt the most popular team in the history of the NBA but right now I think the Rockets (Yao) are bigger.

and Rockets, in number of people, yes because of China, but in others countries, not at all.
Spurs, thank to TP and Manu (France, Argentina and a lot of european and south american fabs) and all the les, even if not so hyped, should be in the TOP 5 right now IMO.

Bulls have dropped i think, that was mainly because of jordan, but the mania is over now.

From my perspective not even close to the discussion, well the Heat maybe.
I was more in a long term perspective rather than today perspective. I've considered a list basically since NBA is famous all over the world. The international interest for the NBA has really begun with M Jordan this is why I begin the list with the Bulls. Today I don't even think that the bulls are in the top 3.
I've lived in Asia for 12 years. Honestly, the Spurs have a very low profile, if any at all.

In Asia, especially in Hong Kong, Spurs are acutally among the top 5 most popular team.
The bulls, rockets, lakers, celtics are the other 4. Surpisingly, pacers are acutally quite popular as well.
