Howard doesn't take near the punishment Shaq used to. Shaq took as much physical contact as he dished out. There were times he had 2-3 defenders around him swiping at the ball and getting all arm but since he was so big and could easily power through it the refs rarely called it unless it was too obvious.

Going off memory I thought Shaq held off because he didn't want to do it. Watching it again, he wanted to make sure that first shot really counted.
When Shaq wound up the first time - all he could see was the back of Miller's head. Once Miller turned his head so Shaq could see his face, he was letting it rip. But an angel must have stopped Miller from turning and instead he again turned the back of his head to Shaq.
