There's been a lot of PG injuries this season so a lot of back ups have been third stringers. I'd say Patty top 5 Maybe. Top 10 def.
Putting in context how good Patty has been this season: He's currently 12th in PER among PGs, the only one in the top-20 averaging less than 20 MPG. Other PGs that could be considered "top" backups:
Tyreke Evans
Darren Collison
Reggie Jackson
Nate Robinson
All of these have a lower PER (admittedly not the greatest stat in the world) than Patty. And while he is obviously not the 12th best PG in the league, I also think that there are half a dozen teams with starting PGs not playing as well as Mills is for us right now.

Shaun Livingston has been playing pretty decent hasn't he? Reggie Jackson, Patty, Nate, Marshall?
You stole my idea! Well you changed it to 5 but good thread! Mills is one of the best backup PG in the league.
Darren Collison, Shaun Livingston, Reggie Jackson not in no order but these names came to mind.
Statistically and based on on/off advanced metrics(RAPM and it's derivatives), Mills has easily been the best backup PG in the league, and one of the top 3 overall bench players in the league, tbh..
Unfortunately, part of that is due to injuries and Parker's struggles(Parker's on/off advanced metrics are atrocious), but regardless, Mills has been fantastic, let's hope he can maintain this pace..
Just wish he was better on defense, Jackson always has a field day against the Spurs because how horrible both Tony and Mills' defense is. If Parker could get back to the level of defense he played last year we'd be set at the pg position.

I think it is very sad that no one is talking about the downside of Patty's accelerated growth and playing time. Sure he adds offense and energy, but, at what cost?
I will even add the fact that he seems like a stellar guy and class act. BUT.......
