I would disagree with the OP example, however, it is a part of movie history that they did several takes of Ford fighting a guy with his whip that wasn't working as intended. At one point Ford did an impromptu, and pulled his gun. Not in the script, but the movie was made that way.

Both the Carol Brunnett comedy and Sonny and Cher shows were known for changing the scripted performances on the fly. Ancient history however. I miss comedy like that.
I have close to 200 seasons of various TV shows on DVD. I have seen in the special features telling how scripts were unintended at times. I don't remember specific examples, but it happens rather often in Hollywood.
I had a good buzz on, but when Sarah Conner was blowing the out of Arnold (the terminator) with the pump shotgun I was laughing hysterically. Couldn't find the youtube but she just kept blowing big chunks off of him...I think everyone else was grossed out by the special effects.
