The smoke was caused by the lighting in the facility. The lighting was a campfire.

Got my teeth fixed 15 years ago in Progreso when I was a stupid with no insurance. They have entire blocks of dentists over there.
Best dentistry I have ever gotten.
Now I'm here next to Juarez... Thankfully I have medical/dental insurance now. Unions are pretty great.
It's really a tourist town, with, as you said, lots of dentists, veterinary, etc, everything that's expensive 3 blocks away across the border
