What is the usual practice for taking out insurance policies against player injuries? Do teams insure all players or just a select few?

they most definitely insure all the players.

You're probably right about the insurance.

After the Amare injury, maybe players should reconsider wearing eye protection during games. James Worthy and Horace Grant proved goggles aren't a hindrance to successful careers. Physical contact to the face and in particular the eyes is a common occurence playing BB. Maybe its vanity on the part of the players or like most young people they feel a sense of invincibility. In any other work environment with with the potential for physical contact to the face, eye protection would have been mandated a long time ago. Why gamble with your career?

IIRC, the NBA has a bulk insurance policy over every player in the NBA. However, the insurance company can pick a certain number of players (10 or 12, I think) that they refuse to cover.
