It was passed to the US by an intelligence agency of one of the Baltic States. The CIA cannot act domestically against American citizens so a joint counter-intelligence taskforce was created.
The taskforce included six agencies or departments of government. Dealing with the domestic, US, side of the inquiry, were the FBI, the Department of the Treasury, and the Department of Justice. For the foreign and intelligence aspects of the investigation, there were another three agencies: the CIA, the Office of the Director of National Intelligence and the National Security Agency, responsible for electronic spying.
Lawyers from the National Security Division in the Department of Justice then drew up an application. They took it to the secret US court that deals with intelligence, the Fisa court, named after the Foreign Intelligence Surveillance Act. They wanted permission to intercept the electronic records from two Russian banks.
Their first application, in June, was rejected outright by the judge. They returned with a more narrowly drawn order in July and were rejected again. Finally, before a new judge, the order was granted, on 15 October, three weeks before election day.

1. There was MAJOR abuse of outside contractors allowed to run 702 about queries according to the FISC audit that Dan Coates released
2. Mike Rogers saw the MAJOR abuse (75+% non-compliance rate?!?!) and shut down the about queries
3. The Dem paid for Steele dossier was used in part or large part to secure a FISA warrant.
4. The FBI/DOJ was collaborating with Steele/Fusion GPS to compose said dossier

They are actually critical to your accusations. One cannot be spying on the Trump campaign through FISA if Page is the only target and he's not working for the campaign anymore.
So again, I'll ask you for the sources of your claims that there was only one FISA warrant ever granted during the course of the investigation and when it was actually granted.

When did Mike Rogers visit Trump without notifying his superiors? When was Dan Coates sworn in? When did Nunes have the press conference reporting on major abuses of 702?
I guess I'd need to be told how Fusion GPS gets an "analytical assistance" subcontracting gig when they are more a PR firm than anything.
And if the access was shut off in April, what does that have to do with an October FISA warrant targeting a person who wasn't with the Trump campaign?
