Emergency = tax dollars.

Fed liquidity = private money from the Federal reserve into liquidity (inter-bank lending) markets, although technically they putting up fiat money if memory serves. pledging assets on the books. Will spend some time looking at the mechanics now that I try and explain it, I realize my understanding is not as solid as I usually prefer.

Markets seemed to like it. Administration finally doing something decisive other than lying about everything. +10%. Almost certain to plummet as bad news trickles in over the weekend, IMO.

apparently it was in response to a liquidity crisis. The treasury note was crashing, almost at 0 on the 10 year note. Banks were worried about the crash creating a financial crisis (then they might stop lending, fearing for a run on banks/ATMs/more liquidity contagion). Call it QE4, 5 or whatever.

In many states, testing rules are so strict that doctors may not notice a community outbreak until it’s too late.

But the majority of Americans still cannot get tested, as interviews with doctors, patients, and dozens of state public-health officials reveal.

3/ The current situation is difficult to imagine and numbers do not explain things at all. Our hospitals are overwhelmed by Covid-19, they are running 200% capacity

4/ We’ve stopped all routine, all ORs have been converted to ITUs and they are now diverting or not treating all other emergencies like trauma or strokes. There are hundreds of pts with severe resp failure and many of them do not have access to anything above a reservoir mask.

5/ Patients above 65 or younger with comorbidities are not even assessed by ITU, I am not saying not tubed, I’m saying not assessed and no ITU staff attends when they arrest. Staff are working as much as they can but they are starting to get sick and are emotionally overwhelmed.

6/ My friends call me in tears because they see people dying in front of them and they con only offer some oxygen. Ortho and pathologists are being given a leaflet and sent to see patients on NIV. PLEASE STOP, READ THIS AGAIN AND THINK.

7/ We have seen the same pattern in different areas a week apart, and there is no reason that in a few weeks it won’t be the same everywhere, this is the pattern:

8/ 1)A few positive cases, first mild measures, people are told to avoid ED but still hang out in groups, everyone says not to panick
2)Some moderate resp failures and a few severe ones that need tube, but regular access to ED is significantly reduced so everything looks great

9/ 3)Tons of patients with moderate resp failure, that overtime deteriorate to saturate ICUs first, then NIVs, then CPAP hoods, then even O2.
4)Staff gets sick so it gets difficult to cover for shifts, mortality spikes also from all other causes that can’t be treated properly.

10/ Everything about how to treat them is online but the only things that will make a difference are: do not be afraid of massively strict measures to keep people safe,

11/ if governments won’t do this at least keep your family safe, your loved ones with history of cancer or diabetes or any transplant will not be tubed if they need it even if they are young. By safe I mean YOU do not attend them and YOU decide who does and YOU teach them how to.

12/ Another typical attitude is read and listen to people saying things like this and think “that’s bad dude” and then go out for dinner because you think you’ll be safe.

13/ We have seen it, you won’t be if you don’t take it seriously. I really hope it won’t be as bad as here but prepare.

The coronavirus is coming to you.
It’s coming at an exponential speed: gradually, and then suddenly.
It’s a matter of days. Maybe a week or two.
When it does, your healthcare system will be overwhelmed.
Your fellow citizens will be treated in the hallways.
Exhausted healthcare workers will break down. Some will die.
They will have to decide which patient gets the oxygen and which one dies.
The only way to prevent this is social distancing today. Not tomorrow. Today.
That means keeping as many people home as possible, starting now.

This is an issue: You only know the official cases, not the true ones. But you need to know the true ones. How can you estimate the true ones?

First, through deaths. If you have deaths in your region, you can use that to guess the number of true current cases. We know approximately how long it takes for that person to go from catching the virus to dying on average (17.3 days). That means the person who died on 2/29 in Washington State probably got infected around 2/12.

Then, you know the mortality rate. For this scenario, I’m using 1% (we’ll discuss later the details). That means that, around 2/12, there were already around ~100 cases in the area (of which only one ended up in death 17.3 days later).

Washington state has today 22 deaths. With that quick calculation, you get ~16,000 true coronavirus cases today. As many as the official cases in Italy and Iran combined.





good charts in there, but basically what it says is if you lock the city down / have people socially distance early and for about a month, that provides hospitals enough time to properly handle serious cases (with intubation, ventilators, etc), and much much fewer people die.

I didn't ask for sympathy for her. I asked for honest thoughts on the merits of her questioning. She is asking the hard hitting questions that every POTUS should stand up and take.

It really is serious because of the social disruption it appears to require in order to avoid overwhelming hospitals. Deaths will occur just like the flu, but we don’t need a spike in infections because this thing is extraordinary in its transmission.

If people in the entire country, not just a few states (decimated by a hurricane) is involved in everyday work, social, provision disruption, it’s a big deal. I have never experienced this. Have you? And it’s basically to avoid a spike that overwhelms our medical facilities. Because this will spread.

I already have 3 friends being tested today thru the end of the week because of symptoms. A pilot and 2 people who travel quite a bit. So depending on the tests their family members will self quarantine on an on, repeat. It will pass but... just keep repeating the above for a few weeks or a month. Family self quarantined but thankfully he does not appear to have extreme symptoms so he will stay at home with them. They are still unsure whether they want to test other family members.

The other 2 did not get tested and are waiting the symptoms out at home. This is all in Texas.

That 900,000 beds is misleading. 65% or so occupancy rate now.

Actual number of available beds is more on the order of 320,000

Be generous and assume that one out of 20 will require hospitalization of some sort. (2% fatality rate would imply more than that requiring urgent care)

320,000/.05= 6.4M.

There is your baseline number of people that can be sick at any one time before the system is swamped. The assumption that all beds are available to all patients is not really a good one.

Big problems will be regional shortages.

Fatalities will be magnified once the capacity in any given region is reached, and doctors cannot treat the ones requiring assistance.

The US is so geographically spread out that there are vast differences in quality of doctors/hospitals/infrastructure that will cause varying amounts of suffering. Anyone who compares the US to South Korea, which is the size of Indiana, glosses over the fact that it is much easier to care for an area so small. If you get sick you just drive from Indy to Fort Wayne and get care, and that's the max anyone would have to drive (<2hrs without traffic).

I feel bad for the restaurant industry, but at this point I'm done eating out or getting fast food until either (1) the new case rate declines enough to give the hospitals breathing room or (2) I get the virus, quarantine and recover (or go to the hospital and not die), and establish I'm negative (hopefully we'll be able to get a test by then). I probably should have stopped it two weeks ago. Especially now that we find out Trump-Pelosi's paid sick leave bill was just for show by exempting companies with 500 or more employees as well as companies with 50 or fewer employees.

I feel bad for people living paycheck to paycheck. If this gets any worse and numbers continue to climb they will have no choice to keep people indoors. I won’t be because of what I do, but people being hard headed and not voluntarily staying indoors when they are sick is what will screw it up for everyone else.
