While in the US restaurant, fast food, gas station, grocery, WalMart, etc workers are going to be working sick to survive.

Shared today from a physician colleague, Dr. Daniele Macchini. He is an ICU doc in Bergamo, Italy, which is just northeast of Milan. They are currently on lockdown.
“After much thought about whether and what to write about what is happening to us, I felt that silence was not responsible. I will therefore try to convey to people far from our reality what we are living in Bergamo in these days of Covid-19 pandemic. I understand the need not to create panic, but when the message of the dangerousness of what is happening does not reach people I shudder.
I myself watched with some amazement the reorganization of the entire hospital in the past week, when our current enemy was still in the shadows: the wards slowly 'emptied', elective activities were interrupted, intensive care were freed up to create as many beds as possible.
All this rapid transformation brought an atmosphere of silence and surreal emptiness to the corridors of the hospital that we did not yet understand, waiting for a war that was yet to begin and that many (including me) were not so sure would ever come with such ferocity.
I still remember my night call a week ago when I was waiting for the results of a swab. When I think about it, my anxiety over one possible case seems almost ridiculous and unjustified, now that I've seen what's happening. Well, the situation now is dramatic to say the least.
The war has literally exploded and battles are uninterrupted day and night. But now that need for beds has arrived in all its drama. One after the other the departments that had been emptied fill up at an impressive pace.
The boards with the names of the patients, of different colours depending on the operating unit, are now all red and instead of surgery you see the diagnosis, which is always the damned same: bilateral interstitial pneumonia.
Now, explain to me which flu virus causes such a rapid drama. And while there are still people who boast of not being afraid by ignoring directions, protesting because their normal routine is 'temporarily' put in crisis, the epidemiological disaster is taking place. And there are no more surgeons, urologists, orthopedists, we are all just doctors who suddenly become part of a single team to face this tsunami that has overwhelmed us.
Cases are multiplying, they arrive at a rate of 15-20 admissions per day all for the same reason. The results of the swabs now come one after the other: positive, positive, positive. Suddenly the E.R. is collapsing.
Reasons for the access always the same: fever and breathing difficulties, fever and cough, respiratory failure. Radiology reports always the same: bilateral interstitial pneumonia, bilateral interstitial pneumonia, bilateral interstitial pneumonia. All to be hospitalized.
Someone already to be intubated and go to intensive care. For others it's too late... Every ventilator becomes like gold: those in operating theatres that have now suspended their non-urgent activity become intensive care places that did not exist before.
The staff is exhausted. I saw the tiredness on faces that didn't know what it was despite the already exhausting workloads they had. I saw a solidarity of all of us, who never failed to go to our internist colleagues to ask, 'What can I do for you now?'
Doctors who move beds and transfer patients, who administer therapies instead of nurses. Nurses and doctors with tears in their eyes because we can't save everyone, and the vital parameters of several patients at the same time reveal an already marked destiny.
There are no more shifts, no more hours. Social life is suspended for us. We no longer see our families for fear of infecting them. Some of us have already become infected despite the protocols.
Some of our colleagues who are infected also have infected relatives and some of their relatives are already struggling between life and death. So be patient, you can't go to the theatre, museums or the gym. Try to have pity on the myriad of old people you could exterminate.
We just try to make ourselves useful. You should do the same: we influence the life and death of a few dozen people. You with yours, many more. Please share this message. We must spread the word to prevent what is happening here from happening all over Italy.
I finish by saying that I really don't understand this war on panic. The only reason I see is mask shortages, but there's no mask on sale anymore. We don't have a lot of studies, but is panic really worse than neglect and carelessness during an epidemic of this sort?"

The doubling rate of the infection is estimated to be every six days. Lab companies think it'll be eight weeks until the supply of COVID-19 tests starts to meet demand.
By then, approximately 1.8 million people in the US will have been infected.

In the meantime let's hope the antiviral remdesivir clinical trials the CDC is conducting are successful for helping the seriously ill, maybe China will come out with real data on how chloroquine is working on the larger trial. Chloroquine would be ideal for less wealthy nations if it works, they aren't going to be able to afford remdesivir (intravenous only).

So the Manhattan lawyer is a good case study. He's in critical condition. His wife and 2 kids are positive but she says they wouldn't even know they were infected because they feel fine.

Two points here: if the extent of the spread of the virus is undermeasured, the overall death rate will look higher; underdiagnosis may also cause higher mortality, cases will be missed, or treatment will start too late to make a difference.
Seems a political emphasis on public health preparedness and prompt, coordinated countermeasures can make a difference on how fast a disease spreads. It might also save a few lives.

The point isn't to stop it, its to slow it down so that the health care system can keep up with it. What is happening in South Korea is an excellent example of this. Its akin to flood water management. You can't stop the flood waters from eventually making it to the ocean. That's simple physics. Its going to pass through. What you want, is to manage the flow of that water to keep it from overwhelming the flood controls you have in place. You don't want it to all come at once and overtop levees so you control the flow, slow it down, and manage it as much as possible.
Eventually most people in this country are going to get the virus. Its almost a certainty. It certainly can't be stopped. But that doesn't mean that we should all hold Coronva Virus Parties to get it over with. Letting the disease run rampant and spread as fast as possible is going to mean that our healthcare system will be overwhelmed and that 0.7 fatality rate will be much higher. That rate isn't some static number that is a law that can't be changed. Its dependent on the care available. Simple bacterial infections don't kill people in this country because we have antibiotics. Suppose we didn't, do the same mortality rates apply? Of course not. The mortality rate in SK is low because they have a much better healthcare system than we do and because they're controlling the spread amazingly well. Downplaying a .7% mortality rate is pretty insane as well but what do you expect from certain people.
The good thing for the US is that we can see what they're doing and put it into play. The bad thing for the US is that there's zero indication that the federal government is interested in pushing social distancing. Trump literally said yesterday in his press conference that he wanted people to travel. The good thing for the US is that these situations are currently happening in places with good leadership at the state and city level that understand what a fucking clusterfuck the feds are currently and are taking it into their own hands. But this won't happen everywhere.
SXSW being cancelled looked to some like an overreaction when it happened. Lets see how many think that in 2 weeks to 1 month.

My son worked summers as a waiter and tips were the only way it made it half way worthwhile. Granted it’s a crappy, exploitative system but fact is it is what it is and not a power trip by the customer but an acknowledgement to the person waiting on you that that’s how they make their living and you’re generous enough to help. Question wasn’t directed specifically at you but everyone including those calling for equality. Do they walk the walk or just talk the talk.

Turns out they’ve just had their third case, traveler from Italy, local who traveled in the states and one with no known exposure. Ain’t nowhere safe, disease free or going to be safe, disease free.

Damn Michael Osterholm was on Joe Rogan's show and made it sound like the virus being slowed by warming weather is a pipe dream since he says it didn't do shit to other coronaviruses like SARS and MERS. Kind of sounds like 500,000+ dead in the US is inevitable.

Hopefully they start giving remdesivir under compassionate use and it works. They gave it to one of the very first guys put in qt when he started going south and he improved overnight.
At some point there will be enough recovered to get enough plasma with antibodies available to use on the very sick. That's what China did.

Maybe for the panic. Maybe much sooner as I do believe it's here in the SA community now.
For healthcare system to be overwhelmed down here it will be longer 6-8 weeks I think unless high temps give us some relief.
Italy must have a different strain, 1st world healthcare and they are dropping like flies. Interestingly the Manhattan lawyer brought his in from Italy and now they have the national guard moving into block that area. Maybe we aren't being told everything.

Strange number. Either they were testing a much wider range of patients, symptomatic and asymptomatic or they really are a super race to get that mortality rate.
